HILL COUNTRY DOODLES HEALTH GUARANTEE

Hill Country Doodles and ________________________, hereinafter referred to as “adoptee”, shall enter into this agreement on ______day of______________, 20____. Hill Country Doodles states that to the best of our knowledge, our Sires and Dams are free from any and all hereditary health conditions. We therefore offer with this guarantee a 26 month guarantee to cover the hips and eyes of our puppies based upon the following conditions.

Purpose of Agreements

The purpose of this agreement is to state the terms and conditions under which Hill Country Doodles will provide a puppy or adult doodle to the adoptee. Therefore in consideration of the mutual promises herein contained, the parties hereto agree as follows:

Services Provided

Hill Country Doodles will provide a puppy or adult doodle in good health, with all necessary shots and worming done for an animal of its age when delivered or picked up.

Examination Required

The Doodle puppy or adult is to be checked by a veterinarian of the adoptee choice for any diseases or parasites within 72 hours after pick-up from or delivery by Hill Country Doodles or this guarantee will be considered null and void.

Shots & Worming

If the Doodle picked up or delivered is a puppy, it must, at adoptee expense be given a 7 way booster at 12 weeks of age, a Rabies Vaccination by 16 weeks of age, monthly Heartworm treatment started by 16 weeks of age and either spayed, if female, or neutered, if male, by the age of 6 months or the entirety of this contract will be considered null and void.

Guarantee & Replacement Policy

• Against hip dysplasia until the age of 26 months and upon certification of dysplasia by either the Penn HIP program or OFA (Orthopedic Foundation of America) certification of moderate or severe hip dysplasia. As soon as this information is sent to Hill Country Doodles in writing with a copy of the report, the puppy or adult must be returned to Hill Country Doodles at the adoptee expense and another puppy from the next available litter of the same Goldendoodle grade and sex (mini, F1b & etc.) will be given or shipped to the adoptee at the adoptee expense. Sole ownership of the original puppy or adult returned to Hill Country Doodles will revert back to Hill Country Doodles.

• Against cataracts of the eye until the age of 26 months and upon certification by CERF (Canine Eye Registration Foundation) that cataracts are detected in one or both of the puppy or adult Goldendoodle’s eyes; this information must also be sent to Hill Country Doodles in writing with a copy of the report and the puppy or adult must be returned to Hill Country Doodles at the adoptee expense and another puppy from the next available litter of the same Goldendoodle grade and sex (mini, F1b & etc.) will be given or shipped to the adoptee at the adoptee expense. Sole ownership of the original puppy or adult returned to Hill Country Doodles will revert back to Hill Country Doodles.

• If the adoptee chooses not to return the puppy or adult, adoptee may keep the puppy or adult and receive a refund of the original purchase price paid. Hill Country Doodles must have the appropriate documents in writing from the above listed organizations before any refund will be given.

• All shots and worming must be done by a licensed Veterinarian and written proof must be presented of these shots and worming before a puppy or adult can be returned to Hill Country Doodles. Parasite checks should be performed by a certified, licensed Veterinarian not less than two times per year and under no circumstances will adoptee veterinarian expense be refunded in connection with the sale of this puppy or adult. The sole and exclusive remedy for this guarantee is listed above in A, B & C of this guarantee.

Extra Provisions

This agreement shall be governed exclusively by the laws of the county of Travis, State of Texas and venue is contractually agreed to be in Travis County, Texas for all purposes.

If any provision of this agreement is illegal or invalid for any reason, such illegality or invalidity shall not affect the validity or enforceability of the remainder of this agreement.

No modification or alteration of the terms of this agreement shall be binding unless in writing, dated subsequent to the date of this agreement, and executed by the parties.
